EMERGENCY MEDICAL AUTHORIZATIONS:
Please thoroughly complete and update Emergency Medical
Authorizations (EMAs). Having the most recent and accurate phone numbers
and medical information is very important for the health and safety of
your child.
DIET ORDERS:
A physician's order is necessary for ALL food allergies. A new order is required yearly for the cafeteria and the nurse. *Forms are available through the Health Office.
MEDICATION ORDERS:
A physician's order is necessary for ALL
medications, prescription or non-prescription. A new order is required
yearly for the nurse. Medications will be kept locked in the Health
Office. *Forms are available through the Health Office.
IMMUNIZATIONS:
State Law requires that children have COMPLETE
immunizations in order to attend school. NM Statue (6.12.2.8.NMAC): the
school nurse is responsible for adherence to this health regulation. Disenrollment is required for those non-compliant.
SERVICES:
Hearing and Vision Screenings are routinely done for
grades PreK, Kinder, 1st, and 3rd. Screening will also be done as
requested from teachers and for IEP/SAT evaluations.
Dental Exams are offered once a year by MIRA Consulting.
Health Education is covered in the classroom-Dental, Germs, First Aid, Puberty
Community Events-Walk & Roll to School, Asthma Allies, UNICEF
